To help you use the services and information provided through this platform safely, and in accordance with the Personal Data Protection Act of Taiwan and its underlying legislative principles, Guardian AI hereby explains its Privacy Protection and Information Security Policy (the “Policy”). Please read the following carefully to understand how we protect your rights and personal information.

1. Scope of Application

This Policy applies to Guardian AI (the “Platform”).

1.2

This Policy applies to the collection, processing, use, and protection of personal data arising from your use of the Platform.

1.3 Exclusions

This Policy does not apply to external websites linked through the Platform, nor does it apply to personnel or organizations that are not commissioned by, affiliated with, or involved in the operation or management of the Platform.

2. Collection, Processing, and Use of Personal Data

To protect your personal data, privacy, and related rights, please read this notice before activating or using the relevant services.

By selecting “Confirm Membership” or any equivalent confirmation option, you agree to exercise your legally granted right of consent electronically. Such electronic consent shall have the same legal effect as written consent.

This notice is provided pursuant to Article 8 of Taiwan’s Personal Data Protection Act and fulfills the statutory notification requirements applicable when personal data is collected.

2.1 Data-Collecting Entity

Guardian AI (the “Platform”).

2.2 Purpose of Collection

The Platform collects personal data for the purpose of providing, administering, and explaining health-related services.

2.3 Categories of Personal Data Collected

2.3.1 Identifying information

Name, address, and telephone number.

2.3.2 Personal characteristics

Date of birth.

2.3.3 Social relationships

Relationship to the person receiving the examination or service.

2.3.4 Lifestyle-related information

Responses concerning lifestyle habits.

2.3.5 Consumer-related information

Responses concerning purchasing or consumption habits.

2.3.6 Health records

Reasons for undergoing a health examination or assessment.

The above categories may correspond to the personal data classification codes C001, C003, C011, C024, C111, and other applicable categories under Taiwan’s Personal Data Protection Act.

2.4

When you merely browse the Platform or download files, the Platform will not actively collect information that directly identifies you.

2.5

During general browsing, the Platform’s servers may automatically record technical and usage information, including your device’s IP address, access time, browser type, pages viewed, and click activity.

Such information is used solely for internal analysis, security management, and service improvement, and will not be publicly disclosed.

2.6

To provide appropriate interactive care and health-related services, certain functions may require you to provide personal information. These functions may include online appointments, registration services, nutritional consultation, satisfaction surveys, and similar services.

2.7

“Personal Data” means information that may directly or indirectly identify an individual, including but not limited to name, date of birth, contact address, and telephone number, as defined under Taiwan’s Personal Data Protection Act.

When you voluntarily provide personal data to the Platform, you consent to the collection, processing, and use of such data within the scope and purposes of the service for which the data was provided.

Unless we have obtained your consent, are required to comply with applicable laws, or are acting pursuant to a lawful order issued by a competent authority, the Platform will not disclose your personal data to third parties or use it for purposes unrelated to the original purpose of collection.

3. Rights of Data Subjects

3.1

The Platform protects your personal data in accordance with applicable laws.

You may choose whether to provide personal data. However, if you decline to provide required information, or if the information provided is incomplete, the Platform may be unable to provide some or all services, or may be unable to provide such services effectively.

3.2 Period, Region, Recipients, and Methods of Processing and Use

3.2.1 Period of processing and use

Your personal data may be processed and used from the date it is provided to the Platform until you request that its use be discontinued or that the data be deleted.

However, where otherwise required by law, or where necessary for health-related or medical operations, the Platform may retain, process, or use such data within the scope permitted by applicable laws and operational necessity.

3.2.2 Region of processing and use

The Republic of China (Taiwan) and countries or regions in East Asia.

3.2.3 Recipients and methods of processing and use

The Platform may process and use your personal data through appropriate methods for the purposes stated above.

Where necessary to perform the relevant services, the Platform may provide personal data to authorized third-party service providers for processing and use within the permitted purpose and scope.

With respect to personal data voluntarily provided by you, you may exercise the following rights under Taiwan’s Personal Data Protection Act:

  • Request a copy of your personal data.
  • Request supplementation or correction.

Please note that the Platform may decline a request where necessary to comply with legal obligations or to perform medical, healthcare, or other legally authorized operations.

4. User Responsibilities and Self-Protection Measures

4.1

When you access an external website through a link provided on the Platform, the privacy and information security policies of that external website will apply.

You are responsible for complying with applicable laws and respecting the legal rights of third parties.

4.2

If your use of an external website linked through the Platform results in, or raises concerns regarding, infringement of another person’s rights, the Platform shall be deemed to have fulfilled its duty to provide reasonable notice and reminders.

The Platform neither authorizes nor assists any conduct that infringes the rights of third parties and shall not be jointly or severally liable for any such conduct arising from your use of an external website.

4.3

If you share a computer with others or use a public computer, you should sign out of your account and close all browser windows after completing personalized or online services to prevent unauthorized access to your information.

4.4

Your use of the internet and the Platform must comply with applicable domestic and international laws.

You are responsible for safeguarding your account credentials, passwords, and any activities or consequences arising from their use.
